Patient-Centered Medical Home
Mission Medical Associates, the physician group practice of Mission Health System, is committed to the long-term goal of qualifying all of its primary care practices as Patient-Centered Medical Homes (PCMH) through the National Committee for Quality Assurance (NCQA). The NCQA has a well-respected and vigorous evaluation and review process for granting PCMH designation.
At Mission, we also define a successful patient-centered medical home as one that meets our Mission Care standards.
